NJCDB Referrals and Requests for Technical Assistance

The New Jersey Center on Deafblindness (NJCDB) is a statewide project that provides expert support for the unique educational needs of students who are Deafblind, their families, and school personnel.  

Process for Referrals

Referrals to NJCDB can be completed by educational professionals, family members or state agencies.

  1. Complete the NJCDB Referral Form.
  2. To determine if NJCDB is able to provide your agency or your family technical assistance, we will ask to have the following forms completed:
    1. Consent to Release Information to NJCDB (Spanish | English)
    2. Census Registry Information
      1. For printing (Spanish | English)
      2. Electronic Form (Spanish | English)

Process for Requesting Technical Assistance and/or Support

Complete the NJCDB TA Request Form  to request information on technical assistance.  After completing the TA request, an NJCDB project staff member will be contact with you.

For TA requests that are connected to an individual student registered with NJCDB, we will confirm the following:

  1. Current Consent to Release Information (Spanish | English)
  2. Updated student/family contact information and school (Spanish | English)

What’s next?

Once all paperwork is completed, an NJCDB project staff member will reach out to collect additional information such as audiological and vision records as well as the etiology of a child’s deafblindness and meet with the individual(s) making the request.  During this meeting, NJCDB project staff member(s) will review the information provided and recommend technical assistance based on the initial request.  NJCDB project staff will also provide additional information on other agencies or organizations that may be helpful.
